Who can pick up an incentive measures cheque following a training course in the CCQ’s regional offices?
- Only people who took a training course held between November 9 and December 13, 2024, and who received or will receive an email specifying the schedule and the location where they can pick up their cheque.
How to pick up your cheque at a regional office
- You must wait to receive an email before going to the regional office.
- You must go to the address given in the email, with two valid pieces of identification, one of which must bear a photograph (Medicare card, driver’s licence, etc.).
How to delegate another person to pick up your cheque
- If you are unable to go to pick up your cheque, you may designate another person by proxy by filling out the Formulaire de procuration. This form, duly filled out, must be handed in when the cheque is picked up, and the person representing you must present two valid pieces of identification, one of which must bear a photograph (Medicare card, driver’s licence, etc.).
